Licensed Practical Nurse in Keene, New Hampshire at Monadnock Family Services

The LPN is a member of the Health Services Team and is responsible for supporting medical providers and clients with medication renewals, documentation, medication injections, outreach calls, triage as appropriate, referrals to community providers, providing a range of treatments including psychosocial rehabilitation, and participating in team meetings and collaboration. Some travel may be required to other MFS facilities.

Reports to: Nursing Services Manager

• Must be NH licensed as an LPN
• At least one year of work experience in an inpatient or outpatient psychiatric setting preferred
• Skills and competence to establish trusting relationships with persons with severe and persistent mental illness and respect for clients’ rights and personal preferences in treatment are essential
• The ability to work collaboratively and respectfully in a team environment is essential
• Must have a valid driver’s license in the state in which the team operates
