Home Health RN or LPN – Come Join Our Team!

Retention bonus of $10,000 paid in quarterly installments during the first year of employment.

We are seeking a compassionate and dedicated home healthcare nurse to provide high-quality care to patients in the comfort of their homes. As a home healthcare nurse, you will play a vital role in improving the health and well-being of our patients, assisting with medical needs, and ensuring that families receive the best care possible. This is a full time, benefitted opportunity that works Monday through Friday with occasional weekend coverage.

Key Responsibilities:

· Administering prescribed medications and treatments according to patient care plans.

· Monitoring vital signs, including blood pressure, heart rate, and respiratory function.

· Assessing patient needs and communicating updates to physicians and family members.

· Providing wound care, catheter care, and other specialized nursing procedures as needed.

· Educating patients and families on care techniques, medication management, and healthy lifestyle choices.

· Documenting patient progress and care in medical records.

· Coordinating with interdisciplinary healthcare teams, including physical therapists and social workers.

Requirements:
  • Graduate of a state-licensed basic nursing program
  • Preparation in Community Staff Nursing through a National League for Nursing approved program
  • With at least one (1) year professional nursing experience under appropriate nursing supervision; or
  • A combination of education and experience that demonstrates the person has acquired the necessary competencies required for the position
  • Possession of a current State of New Hampshire license to practice as a licensed practical nurse
  • Maintain current cardiopulmonary resuscitation (CPR) certification
  • Must be able and willing to learn to utilize a portable electronic field device. Must be able to manage electronic patient health information (ePHI) remote access in accordance with HCS' policies and procedures
  • Sensitivity in human relationships and able to cope with family emotional stress and tolerant of individual lifestyles
  • Skill in using psychological principles in both one on one and group situations
