Key Responsibilities
Clinical Care
- Conduct visits under physician-approved protocols.
- Perform comprehensive patient assessments and document findings.
- Obtain vital signs, medication histories, and treatment updates.
- Perform blood draws and specimen collection as needed.
- Administer injections and other treatments within scope of practice.
- Provide patient education on medications, treatment plans, nutrition, lifestyle changes, and disease management.
- Monitor patients for medication effectiveness and potential side effects.
- Escalate concerns to the medical provider when appropriate.
- Assist with infusion services as new programs are introduced.
Program Management
- Manage the daily operations of the Concierge Wellness Program.
- Develop efficient clinical workflows alongside leadership.
- Ensure every patient receives monthly follow-up.
- Monitor patient compliance and engagement.
- Oversee medical referral coordinators managing refill requests, prior authorizations, and lab follow-up.
- Coordinate care with physicians, pharmacies, specialists, and primary care providers.
- Maintain program quality metrics and patient satisfaction.
- Identify opportunities to improve efficiency and patient outcomes.
Patient Relationship Management
- Serve as the primary clinical contact for concierge patients.
- Build long-term relationships through proactive communication.
- Respond to patient questions via phone, patient portal, and virtual communication.
- Help patients stay accountable to treatment goals.
- Coordinate appointments, laboratory testing, and follow-up care.
Leadership
- Help build and grow the Concierge Wellness Program.
- Train and mentor future staff as the program expands.
- Assist in developing clinical protocols, policies, and standing orders.
- Participate in quality improvement initiatives.
- Collaborate with executive leadership on future service offerings.
Future Program Expansion
- Support GLP-1 medication management.
- Assist with iron infusion services.
- Oversee IV hydration and wellness therapies.
- Participate in remote patient monitoring initiatives.
- Support chronic disease management programs.
- Help expand preventive wellness services.

Additional qualifications:
- Active Licensed Vocational Nurse, EMT-Paramedic, or Licensed Paramedic license, in good standing.
- Minimum of two years of clinical experience; private practice experience preferred.
- Experience working independently while collaborating with physicians.
- Comfortable performing venipuncture and blood draws.
- Proficient in IV starts without ultrasound guidance and strong venipuncture skills.
- Strong patient education and communication skills.
- Excellent organizational and leadership abilities.
- Experience coordinating care across multiple providers.
- Proficiency with electronic medical records.
