RN Home Care Supervisor - Part Time with Benefits 9am-2pm in Niagara Falls, New York at Liberty Home Care

Duties & Responsibilities:

Provide assessment of newly referred patients and assess health needs in home care settingWork with patient to create a patient centered plan of careReassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/familyPrepare written documentation and summaries of the patient’s progress as needed for the continuation of home health servicesProvide supervision and training of PCA/HHA staff

Qualifications:

NYS Licensed Registered Professional NurseMinimum of Associate's Degree requiredOne year Nursing experience requiredHome care experience a plus
