Southern Highlands Community Mental Health Center is seeking a dedicated and compassionate Case Manager/Care Coordinator Level 2 to join our Office-Based Medication-Assisted Treatment (OBMAT) Program.

This position provides comprehensive care coordination and individualized support to individuals receiving treatment for substance use disorders. The Case Manager/Care Coordinator works closely with clients, treatment providers, and community partners to remove barriers to care, promote recovery, and help individuals achieve long-term stability and independence.

• Provide comprehensive case management and care coordination for individuals participating in the OBMAT Program.

• Assist clients in accessing behavioral health, medical, social, educational, housing, transportation, employment, and other recovery-related services.

• Advocate for clients and help address barriers to treatment and recovery.

• Collaborate with treatment providers and community organizations to coordinate services.

• Monitor client progress and ongoing service needs.

• Develop and implement individualized, person-centered treatment and recovery plans.

Applicants must meet one of the following qualifications:

• Bachelor's or Master's degree in a human services-related field, as recognized by the West Virginia Board of Social Work.

• Licensed Psychologist with a Master's or Doctoral degree from an accredited program.

• Licensed Registered Nurse (RN).
