The Opportunity:

Are you an experienced Nurse Practitioner who still loves patient care — and is ready for broader impact through leadership?


Pine Park Health is hiring a Clinical Director, Nurse Practitioner to lead and develop our Bay Area clinical team while staying connected to practice. You’ll coach a small team of NPs and PAs, own provider performance and clinical quality, help hire and grow the team, and continue caring for older adults in senior living communities.

You do not need a Director title today. Lead NPs, APP Leads, NP/APP Managers, Clinical Managers, Clinical Supervisors, and similar leaders are strongly encouraged to apply.


Compensation & Benefits

  • Total target compensation: $200,000-$225,000 (base + 10–15% performance bonus)

What You’ll Do:


Lead a clinical team

  • Lead, coach, and develop a small team of NPs and PAs
  • Spend regular time in the field alongside providers — observe care, coach in real time, and stay close to day-to-day work
  • Hold 1:1s; give ongoing feedback and professional development
  • Manage performance against clinical and operational expectations (visits, documentation timeliness, care-gap closure)
  • Optimize schedules, panels, and coverage across the market
  • Hire, onboard, train, and develop new clinicians
  • Lead quality improvement and strengthen clinical workflows
  • Partner with clinical, operations, growth, and other cross-functional leaders

Build community partnerships

  • Build relationships with senior living community leaders and staff
  • Serve as a clinical leadership point of contact for partners
  • Support new community launches and growth of existing partnerships
  • Resolve clinical and operational issues while keeping partnerships strong

Stay connected to patient care

  • Provide primary care to older adults in senior living communities
  • Deliver Welcome Visits, Care Plans, AWVs, assessments, medication management, and follow-up care
  • Support post-discharge transitions from acute or SNF settings
  • Respond to urgent medical and behavioral health needs
  • Provide coverage when teammates are out
  • Help prevent avoidable ED visits and hospitalizations

What We’re Looking For

Required

  • Active California Nurse Practitioner / APRN license
  • Master’s degree in Nursing
  • Meaningful independent NP practice experience
  • Demonstrated experience leading or managing clinicians (Lead NP, APP Lead/Manager, Clinical Manager/Supervisor, or similar — Director title not required)
  • Experience giving performance feedback and holding teammates accountable
  • Strong clinical judgment; able to assess, care-plan, and manage chronic conditions independently
  • Comfort working autonomously in a fast-paced, multi-site environment
  • Willingness to travel regularly across the Bay Area as part of leading the team

Preferred

  • Primary care, geriatrics, internal medicine, complex/chronic care, home-based or senior care, population health, or value-based care experience
  • Experience leading NPs, PAs, or other APPs
  • Experience hiring, onboarding, coaching, and conducting performance reviews
  • Experience managing teams against measurable goals
  • Family or Adult-Gerontology NP specialization
  • Active unrestricted DEA
  • California 103 NP designation
