Job Description

Public Health Nurse

Position Summary

The Public Health Nurse (PHN) delivers culturally responsive, community-based nursing services in the Pueblo of Acoma community. The PHN focuses on health promotion, disease prevention, case management, and community outreach while honoring tribal sovereignty, cultural practices, and traditional healing pathways.


Key Responsibilities


1. Community Nursing & Outreach

- Provide home visits, wellness checks, screenings, and chronic disease support.

- Deliver health education on immunizations, maternal/child health, diabetes, nutrition, and environmental health.

- Support community events, health fairs, and program outreach.


2. Clinical & Public Health Services

- Administer vaccines, medications, and point-of-care testing.

- Participate in communicable disease surveillance, reporting, and follow-up.

- Support reproductive health, STI education, and family planning services (within scope).


3. Case Management & Care Coordination

- Manage caseloads for high-risk individuals, chronic diseases, and vulnerable families.

- Coordinate with tribal programs, behavioral health, CHRs, and schools.

- Advocate for patient needs and support culturally aligned care plans.


4. Cultural & Community Collaboration

- Build trusted relationships with elders, tribal council, cultural leaders, and families.

- Incorporate cultural practices and traditional knowledge when appropriate.

- Promote trauma-informed care and cultural humility.


5. Emergency Preparedness & Response

- Assist with emergency response plans, vaccination campaigns, infection control, and public health emergencies.

- Provide education on preparedness specific to rural and tribal contexts.


Minimum Qualifications

- RN license required.

- Experience in community health or public health preferred.

- Knowledge of Indigenous health systems, cultural practices, and tribal sovereignty.
