Location: Bend, Oregon - In Person

Status: Part-Time (Approximately 20 hours/week)

We are seeking a passionate and mission-driven Client Care Coordinator to help expand the impact of Shepherd’s House Ministries across all sites and advance this life-changing work.

The Client Care Coordinator is accountable for building trusting relationships with residents and helping them navigate healthcare, overcome barriers to care, and make progress toward their individual health goals. Working under the supervision of the RN Supervisor, this role provides consistent, person-centered support through health education, advocacy, resource navigation, and care coordination while maintaining appropriate professional and clinical boundaries.

Essential Duties and Responsibilities:

  1. Build trusting, respectful relationships with shelter residents through consistent, nonjudgmental, trauma-informed, and person-centered engagement.
  2. Help residents identify healthcare needs and goals and connect with appropriate medical, behavioral health, substance use, dental, vision, and other supportive services.
  3. Support residents with scheduling, preparing for, attending, and following up on healthcare appointments, hospital visits, referrals, and recommended services.
  4. Help residents navigate healthcare systems and overcome barriers related to insurance, benefits, transportation, medical records, identification, pharmacy access, and other basic needs.
  5. Coordinate communication and services among residents, shelter staff, healthcare providers, and community partners while maintaining appropriate confidentiality.
  6. Help residents understand healthcare information, prepare questions for providers, and reinforce health and wellness education provided by licensed professionals.
  7. Identify and promptly communicate observable health, safety, medication, behavioral health, or other concerns to the RN Supervisor or appropriate healthcare professional.
  8. Connect residents with preventive care, behavioral health, substance use treatment, harm-reduction, overdose-prevention, and other community resources as appropriate.
  9. Maintain accurate and timely documentation of resident contacts, referrals, services, follow-up activities, and applicable program outcomes.
  10. Collaborate with nurses, case managers, peer support staff, shelter staff, healthcare providers, and community partners while maintaining professional boundaries and working within the role’s established non-clinical scope.

Qualifications:

Education & Experience:

Required

  1. High school diploma or equivalent.
  2. Completion of a recognized Peer Support training program, or ability to complete required training within the specified timeframe.
  3. Experience working with individuals experiencing homelessness, poverty, healthcare access barriers, behavioral health challenges, substance use, or similar barriers.
  4. Strong interpersonal and communication skills, including the ability to build trusting relationships with diverse populations, maintain professional boundaries and confidentiality, document services accurately, and work effectively both independently and as part of a multidisciplinary team.

Preferred

  1. Active Oregon Health Authority (OHA) Community Health Worker Certification, and/or experience with care coordination or healthcare navigation.
  2. Experience in a homeless shelter, medical respite, healthcare, behavioral health, or social service setting, including familiarity with local healthcare and social service resources.

Physical Requirements/Working Conditions:

  1. Work is performed primarily in a low-barrier shelter and community environment, with frequent interaction with individuals experiencing homelessness, trauma, behavioral health challenges, substance use, and complex needs.
  2. Ability to remain calm, professional, and responsive during behavioral disruptions, crisis situations, and emotionally intense interactions.
  3. Ability to stand, walk, sit, bend, reach, kneel, crouch, and navigate stairs or uneven surfaces throughout shelter and community settings for varying periods during a shift.
  4. Ability to lift, carry, push, or move up to 35 pounds, with or without reasonable accommodation.
  5. Possible exposure to infectious diseases, bodily fluids, and biohazards, requiring adherence to established infection-control and safety protocols.
  6. Ability to travel between organizational sites, healthcare facilities, and community locations and respond appropriately to emergency or crisis situations as required by the position.
