East Arkansas Family Health Center is seeking a compassionate, organized, and dedicated Registered Nurse (RN) Case Manager to join our healthcare team at our Osceola, Arkansas location. This full-time position is responsible for coordinating patient-centered care, assessing patient and family needs, developing and monitoring individualized care plans, and connecting patients with appropriate healthcare, psychosocial, and community support services.
The ideal candidate will have strong communication, clinical assessment, and care coordination skills, with a commitment to improving patient outcomes and ensuring continuity of care. This position works collaboratively with healthcare providers and other professionals to support our Patient-Centered Medical Home model of care.

Coordinate and monitor patient assessments, treatment planning, and follow-up care to ensure timely and appropriate services.
Collaborate with healthcare providers to develop individualized patient care plans.
Evaluate, facilitate, and monitor patient services and progress toward established goals and objectives.
Regularly reassess patient needs and modify care plans as necessary to support continuity of care.
Identify preventive care needs and communicate reminders to providers regarding immunizations, Pap smears, screenings, and other recommended testing.
Schedule appointments for preventive services, diagnostic testing, and referrals as needed.
Establish and maintain accurate follow-up and tracking logs for hospitalizations, emergency room visits, referrals, and other program requirements using eClinicalWorks (eCW) and Aledade.
Assess patient needs and connect individuals and families with appropriate social service agencies and community resources.
Maintain an updated list of community resources available to patients and staff.
Handle telephone encounters, written correspondence, voicemail messages, referrals, and diagnostic appointment scheduling in accordance with established procedures.
Document patient information accurately in the electronic medical record (EMR) in accordance with Patient Care: General Procedure guidelines.
Maintain current CPR certification, TB testing, and required professional licensure.
Demonstrate proficiency in assigned clinical procedures and job-related skills identified during onboarding, orientation, and training.
Perform assigned clinical duties, which may include injections, pulmonary function testing (PFT), oral and nasal swabs, hearing and vision examinations, and other procedures within demonstrated competency.
Complete required temperature documentation, operate and download data loggers, and maintain crash cart logs as assigned.
Complete required medication quizzes and medical proficiency assessments.
Follow established infection control procedures, including proper sharps disposal and cleaning of examination rooms and equipment.
Maintain patient confidentiality and professional boundaries while providing respectful and compassionate care.
Participate in continuous quality improvement activities, including data entry, reporting, meetings, and other quality management requirements.
Participate in the planning and implementation of community outreach events.
Notify supervisors of unusual occurrences, patient complaints, or barriers affecting the completion of assigned duties.
Ensure compliance with HIPAA, OSHA, corporate compliance requirements, and organizational safety and infection control policies.
Complete peer review requirements after six months of employment.
Remain informed of organizational policies, procedures, and operational changes.
Participate in ongoing training and professional development to maintain and improve job-related knowledge and skills.
Demonstrate professionalism, accountability, teamwork, and a commitment to continuous improvement.
Perform other duties as assigned.
Associate degree or diploma in registered nursing.
A minimum of two years of experience as a Case Manager is preferred.
A working background and/or educational background in case management services is preferred.
Ability to interact effectively with other healthcare professionals.
Knowledge of how to document and coordinate program development goals.
Thorough understanding of the patient population served and the goals of the case management program.
Strong understanding of ethical principles, confidentiality, and professional boundaries.
Ability to effectively explain program services and requirements to patients.
Sound judgment regarding safety in both clinical and community settings.
