Job Description
Position Summary:
The Licensed Practical Nurse (LPN) functions as a house-wide clinical resource supporting RNs, Rehab Technicians, Behavioral Attendants, and the interdisciplinary care team in the delivery of safe, effective, and patient-centered care.
Provides direct and indirect nursing care to adolescent, adult, and geriatric patients with complex rehabilitation, neurologic, and medical-surgical needs, including BI and SCI populations, in accordance with the Colorado Nurse Practice Act and under the direction of an RN, physician, or other authorized provider.
Serves as a visible, house wide resource or otherwise directed by supervisor for Rehab Technicians and RNs by providing real-time clinical support, guidance, task assistance, and escalation of patient care concerns.
This role requires a strong on-unit presence, frequent rounding, and excellent communication with staff to promote continuity of care, patient safety, and timely problem resolution.Provides safety, structure and physical care in a consistent manner to patients with physical impairments, impaired reasoning and thinking skills and/ or behavior issues.
Location: 3425 S Clarkson St, Englewood CO 80113
Shift: Monday-Friday; 11 am-23:30 pm MST (11 am- 11:30 pm MST)
Compensation: $22.67–$32.65; plus applicable differentials for the evening shift

Position Requirements:
- Licensure/Certification/Registrations:
- Current Colorado LPN license requied
- BLS, through AHA for the Health Care Provider certification required.
- Education:
- Graduate of a nursing program from an accredited college or university required.
- Graduate of a CCNE or ACEN accredited commission of nursing required.
- Experience:
- Demonstrated experience initiating and maintaining intravenous (IV) access, ensuring patient safety and adherence to clinical protocols required.
- 2 years of LPN experience preferred, ideally in rehabilitation, acute care, medical-surgical or neuroscience setting.
Skills & Abilities:
- Knowledge of practical nursing principles, patient safety, and infection prevention required.
- Ability to collect patient data, observe and report changes in condition, and communicate findings promptly to the RN in support of the nursing plan of care.
- Excellent organizational and customer service skills.
- Ability to manage multiple priorities and respond to changing unit needs.
- Strong interpersonal and communication skills with the ability to serve as a trusted clinical resource across units.
- Ability to provide direct patient care and perform delegated nursing interventions within LPN scope of practice.
- Knowledge of medication administration, treatments, and documentation per policy and delegation.
- Ability to maintain confidentiality at all times.
- Demonstrated problem-solving, teamwork, and professional judgment.
Essential Functions:
- Provide direct patient care under the direction of the RN and in collaboration with the interdisciplinary team.
- Act as a house-wide support resource by assisting RNs and Rehab Techs with patient care needs, clinical tasks, and prioritization during high-acuity or high-volume periods.
- Utilize a consistent, patient-centered, and holistic approach to care.
- Assist patients with activities of daily living, including bathing, dressing, mobility, transfers, toileting, and nutrition.
- Administer medications and treatments as delegated and in accordance with scope of practice and policy.
- Demonstrate effective time management and prioritization while maintaining a visible presence on the unit(s).
- Provide guidance and direction to ancillary nursing personnel as appropriate.
- Document patient care, interventions, and observations accurately and in a timely manner per policy.
- Communicate changes in patient condition promptly to the RN.
- Collaborate with the interdisciplinary rehabilitation team to support patient goals and outcomes.
- Educate patients and families as delegated, reinforcing RN-led education and care plans.
- Use safe patient handling techniques and equipment to prevent injury.
