Established in 1926, Family Service League is a social services agency transforming lives and communities through mental health, addiction, housing, and essential human services with more than 60 social service programs in over 20 locations.

SCHEDULE: Monday to Friday 9 am to 5 pm

SUMMARY: Family Service League is seeking a full-time Case Manager for our Long Island Sound Senior Care program in Greenport, NY. The Case Manager provides comprehensive coordination of health, social and financial services to senior citizens in Greenport.

RESPONSIBILITIES:

  • The Case Manager conducts intakes, assessments, service planning and follow up.
  • Develop and facilitate recreational, educational and social activities.
  • Provide access and help navigate systems and programs.
  • Obtain all proper consent forms to coordinate care.
  • The Case Manager works with government and community agencies.
  • Ensure clients receive needed services, including provision of transportation to appointments.
  • Conduct program outreach.
  • All other duties as assigned.

QUALIFICATIONS:

Associates Degree Required, Bachelor’s Degree in Human Services, Social Work, Sociology or related field preferred.

At least five years’ experience assessing and linking individuals to health related services and programs required.

Excellent interpersonal and verbal and written communication skills required.

Proficient computer skills, including Microsoft Office required.

Bilingual in Spanish preferred.

PHYSICAL REQUIREMENTS:

This position requires sitting for long periods of time, travelling to conduct home visits and transporting donations and program supplies.
