Job Description
The Population Health Nurse ensures continuity of care and services for patients of the organization. The individual will utilize the nursing process to transition patients from the acute setting to the next appropriate site of care with emphasis on promoting continuing healing and preventing readmissions. The individual communicates with patients about their experience as an inpatient and ascertains patient’s condition post discharge. The individual will collaborate with primary care providers on managing chronic conditions, advocating for effective health and well-being. The individual will work with third party payers to identify and close care gaps, assisting providers in meeting their quality initiatives. This individual must have the ability and willingness to work as a team. This individual must maintain effective communication within departments, supervisors, providers, and coworkers. This individual is required to interact appropriately with administration, administrative staff, physicians and other provider staff as well as supervisory staff.
Requirements:
Missouri LPN or RN license in good standing
BLS
One to three years' experience in transitional care management
