CPESN Program Support Lead - Pharmacy Care at Home

Position Summary

The Program Support Lead for the Pharmacy Care at Home works with a large subset of CPESN pharmacy participants to support pharmacy providers seeking reimbursement for clinical services provided in a patient’s home. The Program Support Lead manages large and complex care delivery programs across multiple provider types, environments and workflows, including home-based care and pharmacy operations that deliver medications to home environments. The Program Lead will ensure that PCAH effort will be successful on its own, but also synergistic with parallel efforts to provide clinical services delivery opportunities to CPESN participating pharmacies. Position requires cross matrixed coordination and execution across multiple organizations, levels of readiness and capabilities, and value based program implementations.

Essential Functions

  • Leads communications, reporting, incentives, and operations best practices for all CPESN participating pharmacies.
  • Leads the development and deployment of social learning communities for all PCAH pharmacies.
  • Leads the operations and best practices webinars for all participants.
  • Ensures that all Pharmacy Care at Home Pharmacies understand -- and access -- all of the opportunities available to them via (Flip the Pharmacy, Max My Sync) Pharmacy Care at Home content in an effort to increase workflow efficiency and reduce inventory and maximize patient outcomes.
  • Promotes CPESN opportunities to non-CPESN PCAH pharmacies and non-CPESN pharmacies.
  • Staff participation in the Network Development and Retention Committee
  • Ensures CPESN PCAH members engage with relevant clinical programs and other clinical activities that advance both PCAH efforts as well as pharmacy sustainability.
  • Assists in CPESN pharmacy recruitment.
  • Assists with recruitment and mentorship of human capital assigned to local network leadership and identifying exemplary CPESN PCAH pharmacies for best practices marketing (e.g., videos, webinars).
  • Serves as the primary subject matter expert for CPESN Staff pharmacy care services delivered in a home environment.
  • Serves in a continuous quality assurance capacity to improve Business goals, Care delivery, and Quality assurance needs for the PCAH program.
  • Works with a Luminary Expert committee to assure quality content and necessary support for PCAH pharmacies.
  • Administers a mentorship program leveraging Luminaries for guidance to those accessing the Long Term Care class of trade.
  • Works with Value Based Contract colleagues to use the network to create offerings that reduce gaps in care for payers.
  • Supervises the generation of social media, newsletters, presentations and other content that celebrate and encourage PCAH pharmacy successes and participation in CPESN Programs.
  • Disseminates content and attend meetings that celebrate PCAH pharmacy successes.
  • Attends industry meetings that enhance the visibility and affiliation of pharmacies into CPESN.

Qualifications

  • Associate’s degree in health care delivery or related field or business management or related field (including project management
  • Certified Pharmacy Technician with Operations, project management, and pharmacy support, including the LTC sector
  • 10+ Years of recent community pharmacy practice
  • 5+ Years of experience in at least one independent pharmacy location, managing the implementation of clinical services programs
  • 5+ Year of Business Development success and strategic growth success
  • 5+ Years of experience leading marketing efforts and strategies for companies in health care sector
  • 5+ years of experience creating and implementing workflow efficiencies and operations implementation in a community pharmacy

Preferred

  • 3+ years of experience retaining pharmacy participation in a group-focused organization across states and cultures
  • 5+ years of experience supporting industry events with content, organization, and recruitment
  • 2+ years of experience leading network level social learning communities and discovery of talents and coordination and leading content development
  • Established record of growth in effective team building and executing strategies to organize providers of care

Knowledge, Skills, and Abilities

  • Excellent written and verbal communication skills, within a cross-matrixed work environment
  • Proven track record of building professional relationships and/or professional networks
  • Strong problem solving and critical thinking skills
  • Leadership ability that conveys to indirect means of work effort not falling under a direct reporting relationship

Working Conditions

  • Routinely there may be some minor physical inconveniences or discomforts in the work setting, including sitting for moderate periods of time.
  • Must be able to utilize office equipment, computer, keyboard, and phone with or without assistive devices.
  • Repetitive wrist motion and occasional lifting/carrying of up to 25 pounds.
  • After hours meetings/phone calls/webinars may occasionally be required.
