Posterity Group LLC is a veteran owned, service-disabled small business, headquartered in Rockville, MD, that specializes in federal government contracting and staffing. We are tasked with staffing VA New York Harbor Healthcare System with Pain Physical Therapists. The Physical Therapist is expected to do the following:

The Physical Therapist would work out of one of the two VAHC CBOCs (Community Based Outpatient Clinics):

  • Assess the patient prior to development of treatment plans and goals, exercising autonomous clinical decision-making through Direct Access.
  • Implement plans in accordance with initial evaluation findings for treatment aimed at prevention, reducing disability or pain, restoring lost function, and increasing participation.
  • Provide therapeutic interventions at each visit, which may focus on: posture, wheelchair mobility, gait, transfers, strength, endurance, balance, coordination, joint mobility and flexibility, functional tasks, and pain neuroscience education.
  • Apply evidence-based behavioral strategies to identify and address fear-avoidance presentations and adapt physical therapy approaches for patients with co-occurring severe mental and physical disabilities.
  • Monitor the extent to which services have met therapeutic goals relative to the initial and all subsequent assessments at each visit.
  • Determine the degree to which improvement occurs and, when appropriate, revise the treatment plan at each visit.
  • Provide education to patients, families, and/or significant others regarding deficits incurred and interventions to reduce the existing deficit, and management of any residual deficit in resumption of life roles in the home and community at each visit.
  • Perform chart review prior to each scheduled appointment and obtain a subjective history of assigned patients.
Requirements:

Physical Therapist(s) shall demonstrate a minimum of one of the following:

• (a) A minimum of 2,000 hours of clinical practice directly in chronic pain physical therapy within the past two to five years; or

• (b) Board Certification in a specialty area recognized by the ABPTS; or

• (c) Completion of a fellowship program in a related specialty; or

• (d) An additional advanced degree in a related field.

Personnel shall also have a minimum of two years of hospital or ambulatory care experience, or equivalent experience in chronic pain management.
