The Opportunity

Pine Park Health is looking for a Full-Time Psychiatric Mental Health Nurse Practitioner (PMHNP) to launch our behavioral health program in San Diego. This is a founding clinical role: you will deliver on-site psychiatric care to older adults living in senior living communities while working directly with our Chief Medical Officer to design, build, and lead the behavioral health function across Pine Park Health. If you are excited about caring for a complex, underserved population, practicing at the top of your license with both medication management and psychotherapy, and shaping a value-based behavioral health model from the ground up, you have come to the right place.

What you'll do

• Provide on-site psychiatric medication management and psychotherapy to older adults living in senior living communities (assisted living, memory care, and independent living) across the San Diego market
• Conduct initial psychiatric diagnostic evaluations and develop person-centered behavioral health care plans, with particular attention to dementia-related behavioral and psychological symptoms, late-life depression and anxiety, and psychotropic stewardship
• Lead gradual dose reduction and deprescribing efforts for antipsychotics and other psychotropics in partnership with primary care teams, consulting pharmacists, and community staff
• Collaborate closely with Pine Park primary care pods — joint case reviews, curbside consults, and interdisciplinary team meetings — to integrate behavioral health into our primary care model
• Partner with the Chief Medical Officer to design and lead the behavioral health function: clinical protocols, referral pathways, documentation and billing workflows, quality metrics, and the roadmap for expanding the program to additional markets
• Work with senior living community leadership and care staff to build behavioral health capability on-site, including staff education on managing behavioral expressions of dementia
• Track and improve outcomes that matter in value-based care: avoidable ED visits and psychiatric hospitalizations, psychotropic polypharmacy, and resident and family experience

Qualifications

• Board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) with an active, unrestricted California license and DEA registration (or ability to obtain prior to start)
• 3–5 years of experience caring for geriatric patients; experience in senior living, skilled nursing, or other long-term care settings strongly preferred
• Skilled in both psychopharmacology and brief psychotherapy for older adults, and comfortable managing behavioral and psychological symptoms of dementia
• Builder's mindset: energized by creating something new, comfortable with ambiguity, and eager to help design the program you will practice in
• Strong orientation toward collaboration and working closely with a multidisciplinary team; values the input of all members of the clinical team
• Strong written and verbal communicator, including with families, facility staff, and non-clinical audiences
• Ability to work with tech-enabled care delivery tools (e.g., project management, 'checklist' management, remote monitoring) used together to improve patient care and team function
• Flexible work schedule with travel within the San Diego area; this role involves driving between partner communities as needed

The base salary range for this role is $135,000 to $160,000 plus generous bonus, equity, and benefits. The final compensation package for each successful candidate will depend on several job-related factors unique to each candidate. These factors may include but are not limited to education, training, skill set, years and depth of experience, certifications and licensure, business needs, internal peer equity, and alignment with geographic and market data.
