Position Profile

The Intake Coordinator role is crucial in facilitating exceptional patient experiences, starting with responding to telephone inquiries and managing incoming calls from patients, their families, and other individuals. This role addresses requests, resolves concerns, and facilitates communication with other members of the healthcare team. This role is responsible for gathering patient information, creating admission and discharge paperwork, providing explanations of policies and procedures, and addressing patient inquiries, concerns, and grievances. This role is responsible for scheduling provider visits.

This role is Hybrid (3 days in office, 2 days from home)

Responsibilities

  • Performs patient outreach to schedule appointments with the provider.
  • Manages phone/email communications effectively and efficiently.
  • Facilitates inbound calls from patients requesting new appointments or change to existing appointments.
  • Maintains and coordinates schedule with provider and patient.
  • Makes necessary adjustments to patient/provider schedules due to reschedule requests or cancellations.
  • Verifies patient demographics and insurance information for accurate charting.
  • Performs staff assignments.
  • Ensures data integrity.
  • Assures paperwork completeness and timelines are met.
  • Performs all other duties as assigned.
  • Maintains professional knowledge and enhances professional skills.
  • Adheres to high standards of personal and professional conduct.

Minimum Qualifications

  • High School diploma.
  • Minimum two years of related work experience including data entry.
  • Experience with Microsoft Office applications, including Excel, Word, and Outlook.

Preferred Qualifications

  • Familiarity with medical terminology.
