About This Position
Position Description: ESSENTIAL DUTIES:
Supervises, delegates, and manages all direct care given to patients by RNs, LVNs and Nursing Aides in an appropriate manner. Ensures continuity of care by appropriate directly, teaching and counseling the support staff
Models a service and solutions oriented attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to patients, visitors and co-workers
Ensure patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards
Maintain good attendance record
Demonstrates appropriate physical assessment skills of patients status. Possesses an adequate knowledge of clinical diseases and disease progression. Actively participate in coordination of all aspects of patients care, in accordance with current professional standards and practice.
Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family
Develop and evaluate the plan of care
Participate in QAPI program and hospice sponsored in-service training
Participate in ongoing interdisciplinary comprehensive assessment
Maintains confidentiality of all patient information, sharing only what needs to be shared with the interdisciplinary team in order for the patient to be adequately cared for
Maintain regular communication with the Director of Business Development, Director of Clinical Operations and Patient Care Manager in order to follow-up for every admission.
Respects patient/family rights by: (knocking on doors before entering the room, addresses patient/family by name, allows patient to make own decisions when appropriate)
Promptly reviews, revises and confirms staff assignments for the following shift, to ensure all patients are assigned to the appropriate staff. Ensures staff are adequately caring for patients during shift and makes appropriate staff changes as needed. Asses the patients clinical condition and productivity of the assigned staff
Completes admissions, transfers and discharges. This includes assessment, documentation, hand off report, ordering of medications/medical equipment as needed and anything else required to ensure proper care and comfort is provided to patient.
Orders medications as needed and counts narcotic at beginning of shift and at end of shift.
Demonstrates ability to perform nursing treatments and procedures as prescribed for patients as needed. Administers those treatments in an appropriate manner as needed
Plans the administration of and/or administers medication with knowledge of actions, contraindications, side effects and desired outcomes.
Ensures that all patients under his/her direct supervision are provided care and services to attain or maintain the highest practicable physical, mental and psychosocial well-being
Provides emotional support and compassionate care to the patient and family. Demonstrates listening skills, offers encouragement when appropriate, adjusts care to the patients physical condition each visit
Obtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.
Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support.
Maintain up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.
Instruct/Educate primary caregivers, volunteers, and employed caregivers to provide care as indicated; Contribute to patient and family counseling and education
Practices Universal Precautions according to OSHA regulations and Faith Hospice policy
Notify the MD, family, administrative staff and direct care providers of resident change of condition and necessary follow up
Assist other nurses including assuming primary responsibility for patients when necessary
Attends and participates in orientation and appropriate in-services and staff meetings to improve knowledge and skills and to maintain competency
Provide appropriate support at time of death. Reports death to appropriate staff. Calls medical examiner to report death and calls funeral home
Complete an incident report as indicated and ensures adequate follow-up is completed and/or incident is resolved
Follows documentation procedures as required by State and Federal regulations. Uses accurate medical terminology
Communicates unresolved work-related problems using chain of command, consistently demonstrates effective problem-solving skills and handles confidential information appropriately
Demonstrates flexibility with a changing workload/caseload, sets priorities accordingly and carries out job assignments in a self-directed manner
Consistently maintains productivity at an acceptable level
Perform CNA duties as needed
Available for holiday, on-call, illness and vacation coverage as needed.
Other duties as assigned by Patient Care Manager
Additional Information: MINIMUM QUALIFICATIONS: Minimum of two years of varied work experience as a professional nurse and preferably one of the two years in an hospice inpatient setting Possession of a valid Texas Registered Nurse License Proficient ability to communicate orally and in writing in the English language Valid Texas Drivers License and reliable method of transportation Ability to work independently Ability to adjust to changing conditions and frequent demands Ability to acquire new knowledge to perform additional or more difficult tasks Ability to maintain a positive, stable disposition in performing, working in association with staff, patients and families Ability to maintain a high level of performance in an environment of frequent demands and changes Ability to meet new and/or stressful situations with the appropriate responses, ability to work effectively as part of a healthcare team, to share information freely, to use and seek supervision and consultation verbally and in writing, and to be able to follow both verbal and written instructions Ability to perform Essential Duties as outlined below Ability to perform work tasks within the Physical Demand Requirements as outlined below Genuine caring for and interest in the care of mature adults, handicapped people and hospice Patients Ability to perform work tasks within the Physical Demand Requirements as outlined below Ability to comply with the Dying Persons of Rights and the Employee Responsibilities Ability to comply with organizational and departmental safety policies and procedures
