About This Position
Hushabye Nursery is an Arizona nonprofit organization with a mission to ‘embrace substance exposed babies and their caregivers with compassionate, evidence-based care that changes the course of their entire lives.’ The organization was founded by Tara Sundem, a neonatal nurse practitioner who witnessed firsthand the challenges of newborns exposed prenatally to opioids and those families trying to care for them. Hushabye Nursery’s innovative model offers a safe and inclusive space where mothers, family members and babies – from conception through childhood – can receive integrative care and therapeutic support that offers each child the best possible life outcomes. Programs will include prenatal and postpartum support groups, inpatient nursery services, and outpatient therapies.
POSITION OVERVIEW: The Case Manager provides comprehensive, client-centered services to pregnant and parenting individuals receiving outpatient substance use support. This role is responsible for assessment, treatment planning, care coordination, trauma-informed service delivery, progress monitoring, and discharge planning. Case Managers work collaboratively with a multidisciplinary team to support recovery, family stability, and healthy outcomes for parents and families. Case Managers are expected to meet the productivity standards outlined by their position in accordance with program and payer requirements.
DUTIES & RESPONSIBILITIES:
Clinical and Case Management Services
- Conduct comprehensive biopsychosocial assessments to identify client needs across behavioral health, physical health, housing, employment, legal, and social determinants domains.
- Develop, implement, and update individualized treatment plans in collaboration with clients
- Provide individual case management and skills training sessions
- Facilitate group sessions as assigned
- Coordinate and document referrals, monitor follow-through, and advocate for access to support (medical providers, other behavioral health providers, child welfare, vocational, housing, transportation, community resources etc).
- Monitor client progress toward treatment goals and document outcomes
- Participate in multidisciplinary treatment team meetings and transition/discharge planning.
- Provide education related to substance use recovery, parenting, prenatal/postpartum health, and life skills
- Support client engagement, retention, and adherence to treatment recommendations
Documentation and Compliance
- Complete all clinical documentation accurately, thoroughly, and within 24 hours of service
- Maintain timely and accurate clinical documentation in compliance with organizational policies and regulatory standards (i.e., AHCCCS, ADHS, and CARF)
- Ensure documentation supports medical necessity and billable service requirements
- Maintain compliance with confidentiality, ethical standards, and professional boundaries
- Participate in audits, chart reviews, and quality improvement activities
Discharge Planning and Continuity of Care
- Develop discharge and aftercare plans in collaboration with clients
- Coordinate referrals to ongoing treatment, recovery supports, and community services
- Ensure continuity of care and appropriate transitions upon discharge
Team Collaboration and Professional Responsibilities
- Participate in supervision, team meetings, and case conferences
- Collaborate with interdisciplinary staff
- Engage in ongoing training and professional development
- Contribute to a trauma-informed, recovery-oriented, and family-centered program culture
Productivity Expectations
- Staff in this role are expected to meet the productivity standards outlined by their position in accordance with program and payer requirements.
- Balance direct service delivery with required documentation and administrative tasks
- Proactively manage schedule to meet productivity standards while ensuring quality care

This position plays a critical role in supporting recovery, parenting capacity, and long-term family stability for clients served by the program.
EDUCATION (MINIMUM REQUIREMENTS) and Experience:
Required:
- Bachelor’s degree in behavioral health, social work, human services, or related field. Equivalent experience as outlined below can serve in lieu of a bachelor's degree.
- 3 years of experience in Social Work or another related field.
- Experience working in substance use treatment or behavioral health settings
- Knowledge of recovery-oriented, trauma-informed, and family-centered care
- Ability to meet documentation, billing, and productivity requirements
Preferred:
- Experience working with pregnant and parenting populations
- Case management or counseling certification or licensure (as applicable)
- Familiarity with Medicaid/AHCCCS or similar payer systems
- Lived experience relevant to recovery or parenting (valued but not required)
