Hours Per Week: 14 Hours

Duties and Responsibilities:

• Provide psychiatric evaluations and medication management for geriatric clients in an outpatient clinic setting, at a clinic location, via telehealth, or in the home, if needed

• To participate with other staff members in the development of a treatment plan based on the diagnostic assessment, to periodically reassess patients’ needs and circumstances and revise treatment plan if appropriate;

• To help coordinate the patient’s care with hospitals and other providers;

• To help with hospitalization when appropriate;

• To conduct seminars and play a part in the education of the team where time permits and the necessity arises;

• To be available to the team for consultations, when necessary.

• Attends and participates in staff meetings and periodic in-service training when available.

• Additional tasks as assigned.

Requirements:

• Valid NYS license to practice in discipline

• Current DEA Registration Number

• Possession of or ability to obtain a NPI number and Medicare Provider Number

• Desire and ability to work collaboratively and engage in quality improvement activities as part of a committed, integrated team

• Excellent interpersonal skills and the ability to communicate effectively and concisely both in writing and orally

• Strong attention to detail with the ability to solve problems; to elicit, comprehend, and synthesize accurate and specific information; prioritize work; to work independently; and to excel in a high-performance culture

• Good writing/record keeping skills

• Basic computer skills

• Familiarity with electronic health records and e-prescribing desired

• Ability to read, write speak and understand English
