JOB TITLE: Case Manager/SOAR
DEPARTMENT: Supportive Housing Services
FLSA: Non-Exempt
LOCATION: Windham
REPORTS TO: Service Coordinator
SALARY: $45,000.00 - $50,000.00
SUMMARY: The Case Manager is responsible for providing comprehensive case management and supportive services to individuals residing in supportive housing and transitional-based programs. The Case Manager conducts intake/annual assessments, develops individualized service plans, coordinates care, and connects clients to essential services and benefits. This position works collaboratively with internal staff and external service providers to promote housing stability, self-sufficiency, health and wellness, and overall quality of life for clients experiencing homelessness, mental health challenges, substance use disorders, physical health conditions, and other barriers. This position requires a 24/7 work phone availability in case of any emergency.
DUTIES & RESPONSIBILITIES:
• Responsible for managing then entire building’s caseload.
• Ability to work with various contracted programming and a diverse population.
• Responsible for completing lease packets and collecting rent.
• Provide ongoing case management, supportive counseling, crisis intervention, advocacy, information and referral services.
• Responsible for completing CE and CHAT Assessments for the Transitional Clients.
• Responsible for assisting transitional and permanently housed clients with educational trainings, employment and housing referrals.
• Assist clients in accessing medical care, mental health treatment, substance use treatment, vocational services, educational opportunities, transportation, legal services, and other community resources.
• Support clients in obtaining identification documents, public assistance, disability benefits, Social Security benefits, and other entitlements. Must have the ability to attend DSS and other meetings with clients as needed.
• Build and maintain positive working relationships with clients through engagement, goal setting, and strengths-based interventions.
• Works closely with various departments at Westhab to ensure compliance and program safety.
• Complete HMIS and other intake assessments, admissions, annual assessments and exit assessments.
• Conduct regular room and unit inspections and address resident concerns related to housing stability and program compliance.
• Report maintenance and property deficiencies to appropriate departments and follow up as needed.
• Maintain accurate, timely, and comprehensive client records, case notes, assessments, service plans, and required reports. Monitor client progress toward established individualized goals and document outcomes appropriately.
• Attends various internal training meetings and client related meetings. Ability to work independently and in a group.
• Assists with the monitoring of monthly rent payments, delinquent accounts and appropriate follow-ups under the direction of the Service Coordinator.
• Work collaboratively with other colleagues to ensure the proper documentation is in case records for each client to meet the requirements of our funders and agency compliance.
• Other work-related duties as assigned.
EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:
A Bachelor’s Degree in Social Work, Psychology, or related field with prior social service experience preferably working with substance abuse, mental health and homeless populations. Bi-lingual (Spanish) a plus. Position requires a valid driver’s license and own vehicle. Ability to drive a Westhab vehicle to transport client as needed or to pick up donations. Detail oriented to complete requirement of files and contract compliance packets. Excellent written and communication skills. Possess strong organizational skills with the ability to meet a demanding workload. Some evenings and weekends as needed. Must have excellent written/verbal communication skills.
