What You'll Do
- Provide professional nursing care in accordance with the Nurse Practice Act under the supervision of a Registered Nurse; promptly report changes in patient condition per agency guidelines
- Promote and maintain a safe environment for clients, families, and coworkers
- Perform all duties in compliance with agency policies, procedures, and professional standards
- Assist in coordinating home care services, attend required meetings, and follow established reporting protocols
- Operate glucometers and INR monitoring devices with basic proficiency
- Deliver basic wound care in accordance with established care plans
- Complete additional duties as assigned
What You Bring
- Graduate of an accredited professional nursing program
- Current Licensed Practical Nurse (LPN) license in applicable state(s)
- At least 1 year of acute care experience or equivalent (community/home health preferred)
- Current CPR certification
- Strong written and verbal communication skills with effective interpersonal abilities
- Experience using electronic medical records (EMR) systems
- Proficient with tablet-based documentation and email/text communication
- Ability to interact tactfully with clients and community members
- Strong organizational and time management skills
- Valid driver’s license, reliable insured vehicle meeting state/agency requirements, and a clean driving record
