Registered Nurse (RN) - Assisted Living

Location: Brooklyn Park, MN (Multiple Locations)

Job Type: Full-Time

Shift: Days

Position Summary: We are seeking a compassionate, skilled, and dedicated Registered Nurse (RN) to join our assisted living community in Brooklyn Park, MN. The RN is responsible for the overall health and well-being of our residents, providing professional nursing care, and overseeing the clinical operations of the community. This role requires a nurse who is a strong leader, a meticulous planner, and a team player committed to fostering independence and dignity for our residents.

Key Responsibilities:

  • Conduct comprehensive initial and ongoing health assessments for all residents to create personalized service plans and plans of care.

  • Monitor and evaluate residents' health status, recognizing and responding to changes in condition in a timely and professional manner.

  • Administer medications and treatments as prescribed by physicians, and ensure accurate and timely documentation.

  • Coordinate with residents' families, physicians, and other healthcare providers (e.g., hospice, home health, physical therapy) to ensure continuity of care.

Clinical Leadership & Oversight:

  • Serve as the primary clinical leader, managing and upholding clinical standards in accordance with the Minnesota Nurse Practice Act and other state and federal regulations.

  • Supervise and provide guidance to Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and other unlicensed personnel.

  • Delegate appropriate healthcare tasks to qualified staff and ensure a robust system for supervision and evaluation of all delegated activities.

  • Participate in quality improvement initiatives and audits to maintain a high standard of care.

Documentation & Compliance:

  • Ensure all clinical documentation is accurate, complete, and up-to-date, utilizing the community's electronic medical record (EMR) system.

  • Process and implement physician orders in a timely fashion.

  • Investigate and document any medication errors or incidents, and implement corrective action plans as needed.

Community & Team Collaboration:

  • Build strong, positive relationships with residents, families, and staff, promoting a culture of compassion, respect, and dignity.

  • Assist in the training and orientation of new clinical staff.

  • Respond to resident requests for assistance and emergency situations promptly and effectively.

Qualifications:

  • Current and unrestricted Registered Nurse (RN) license in the state of Minnesota.

  • Current CPR certification.

  • Minimum of 3 years of experience in an assisted living or related healthcare setting is preferred.

  • Strong knowledge of regulatory compliance and clinical best practices, especially concerning geriatric care and common conditions such as dementia.

  • Excellent communication, organizational, and problem-solving skills.

  • Proficiency with electronic medical records (EMR) and standard computer software, specifically Residex (formerly RTasks).
