Office Location: Remote within North Carolina.

What We’re Looking For
Trillium Health Resources has a career opening for a Care Management Specialist II to join our team! The Care Management Specialist II provides professional-level care coordination services within a managed care environment. The role independently conducts comprehensive assessments, coordinates services across providers and community resources, and facilitates member transitions to ensure continuity of care, exercising judgment in aligning member needs with appropriate resources.
On a typical day, you might:
- Conduct comprehensive assessments of member needs using organizational platforms and available data sources.
- Provide short-term care coordination services, aligning member needs with appropriate services, benefits, and community resources.
- Develop, implement, and monitor individualized care plans in collaboration with members and their support teams.
- Conduct record reviews to identify care gaps and support care planning and service coordination.
- Coordinate services and resources across providers, agencies, and community organizations to address identified member needs.

Qualifications
Required:
- High school diploma or GED and six (6) years of experience working with mental health, substance use, and/or intellectual/developmental disability populations; OR
- Associate's degree and four (4) years of experience working with mental health, substance use, and/or intellectual/developmental disability populations; OR
- Bachelor's degree and two (2) years of experience working with mental health, substance use, and/or intellectual/developmental disability populations; OR
- Equivalent combination of education and experience.
- Must have a valid driver’s license.
- Must reside within North Carolina.
- Must be able to travel within catchment as required.
Preferred:
- Degree in Human Services
- Bachelor’s degree
