As we continue to expand our post-acute care services, we are seeking a compassionate, dependable, and motivated Physician Assistant (PA-C) or Nurse Practitioner (NP) to join our team as a SNFist, providing medical care to patients in skilled nursing facilities.
Position SummaryThe SNFist PA-C or NP will provide comprehensive medical management to patients residing in Skilled Nursing Facilities (SNFs). This position is ideal for a provider who is comfortable managing medically complex adult and geriatric patients and working collaboratively with physicians, nursing staff, rehabilitation teams, specialists, and facility leadership.
The provider will be responsible for evaluating patients, managing acute and chronic conditions, completing follow-up visits, coordinating care, and supporting safe transitions throughout the patient's post-acute stay.
Requirements: Key Responsibilities- Provide medical care and routine rounding for patients in assigned skilled nursing facilities
- Perform comprehensive patient assessments, histories, and physical examinations
- Manage acute and chronic medical conditions within the provider's scope of practice and credentials
- Complete admission evaluations, follow-up visits, and discharge-related care as assigned
- Develop, implement, and monitor individualized treatment plans
- Order and interpret laboratory testing, diagnostic imaging, and other clinically appropriate studies
- Evaluate changes in patient condition and initiate appropriate treatment or escalation of care
- Review and manage medications, including medication reconciliation when appropriate
- Coordinate specialty consultations, referrals, and hospital transfers when medically necessary
- Collaborate closely with physicians, nursing staff, rehabilitation teams, case management, social services, and other members of the interdisciplinary care team
- Participate in care coordination and discharge planning to promote safe transitions of care
- Communicate appropriately with patients, families, facility staff, and members of the healthcare team
- Educate patients and families regarding diagnoses, medications, treatment plans, and goals of care
- Complete accurate, thorough, and timely documentation in the electronic health record
- Maintain compliance with applicable clinical, regulatory, quality, and patient-safety standards
- Provide compassionate, patient-centered care while supporting continuity across the post-acute care setting
- Current and unrestricted California PA or NP license
- Current national certification as a Physician Assistant or Nurse Practitioner
- Current DEA registration, if required for assigned responsibilities
- Current BLS certification
- Strong clinical assessment and decision-making skills
- Excellent communication, documentation, and care-coordination skills
- Ability to work independently while collaborating effectively with supervising/collaborating physicians and facility teams
- Experience in skilled nursing, post-acute care, geriatrics, hospital medicine, internal medicine, or primary care preferred
- Comfortable managing medically complex adult and geriatric patients
