Job Title: CES Case Manager
Location: Mercer County, WV
Position Type: Full-Time
Organization: Southern Highlands Community Mental Health Center

Job Summary

Southern Highlands Community Mental Health Center is seeking a dedicated and compassionate CES Case Manager to support individuals with serious mental illness who have a history of, or are at risk for, involuntary commitment. This role focuses on helping individuals live successfully in the community of their choice while reducing the risk of psychiatric hospitalization.

The CES Case Manager works within Targeted Case Management (TCM) guidelines, maintains an assigned caseload, and provides direct community-based services.

Essential Responsibilities
  • Provide community-based case management services to individuals with serious mental illness and co-occurring disorders

  • Assist clients in accessing behavioral, medical, social, educational, and health-related services

  • Coordinate care among providers and community resources

  • Advocate on behalf of clients to ensure appropriate services and supports

  • Develop and implement person- and family-centered treatment plans

  • Maintain documentation in accordance with TCM and agency guidelines

Education & Experience Requirements
  • Bachelor’s or Master’s degree in a human services-related field (per WV Board of Social Work guidelines), OR

  • Licensed Psychologist (Master’s or Doctoral degree from an accredited program), OR

  • Licensed Registered Nurse

Experience working with individuals with serious mental illness and/or co-occurring disorders preferred.
