Family Medicine Physician or Advanced Practice Provider (NP/PA)

We are seeking a dedicated and compassionate Family Medicine Physician or Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our team. Key Responsibilities:

- Provide comprehensive outpatient primary care to patients of all ages, including preventive, acute, and chronic care management

- Conduct patient assessments, perform physical examinations, and develop individualized treatment plans

- Order and interpret diagnostic tests, prescribe medications, and coordinate follow-up care

- Collaborate with a multidisciplinary team to ensure holistic patient care

- Document patient encounters accurately in electronic health records (EHR)

- Educate patients and their families on health management, disease prevention, and treatment options

- Participate in quality improvement initiatives and clinic meetings to enhance patient care delivery

Skills and Qualifications:

- Medical degree (MD or DO) for physicians, or current licensure and certification as an NP or PA

- Board certification or eligibility in Family Medicine or a related primary care specialty

- Valid state medical license and DEA registration

- Experience working in outpatient primary care settings preferred

- Strong clinical skills, with the ability to manage a diverse patient population

- Excellent communication and interpersonal skills

- Ability to work collaboratively within a team-oriented environment
