POSITION SUMMARY: The Heritage Place Social Service Associate (SSA) serves as a primary advocate for residents, working as part of the Heritage Place Interdisciplinary Care Team. Under the direction of the LTC Administrator or designee, the SSA supports residents in maintaining their highest level of activity and life enrichment in ways meaningful to them.

Additionally, this role oversees resident funding procurement before admission and collaborates with the Heritage Place financial coordination team and resident representatives to ensure ongoing funding throughout their stay. These responsibilities align with federal and state regulations. As a key member of the interdisciplinary team, the SSA also performs other duties within their scope of practice to support high-quality resident care.

REQUIREMENTS:

• Associate's Degree in Social Services
• Progressively responsible experience in Social Services may be substituted in lieu of education on a year-for-year basis
• BLSP (CPR) from the American Heart Association
• Long Term Care experience, preferred
