Position Overview:
We are seeking dedicated and experienced Licensed Practical/Vocational Nurses to join our compassionate team. This role provides the opportunity to work one-on-one with patients in their homes, delivering high-quality, patient-centered care that makes a real difference in their lives.

Responsibilities:

• Conduct comprehensive nursing assessments of patients in their homes, considering physical, psychosocial, and environmental factors.

• Perform skilled nursing procedures and treatments, including wound care, injections, catheter care, and medication administration.

• Monitor patients’ health status and communicate any changes to supervising RNs or healthcare providers.

• Educate patients and caregivers on disease management, medications, and preventive care.

• Provide emotional support and guidance to patients and families.

• Collaborate with an interdisciplinary team to ensure coordinated care.

• Document assessments and interventions accurately using electronic health record (EHR) systems.

• Support care planning, goal setting, and transitions of care.

• Follow infection control and safety guidelines to protect patients and staff.

• Participate in team meetings, case conferences, and quality improvement initiatives.

Qualifications:

• Graduate of an accredited practical/vocational nursing program with a current state LPN or LVN license.

• 2–3 years of nursing experience, preferably in home health, community health, or related fields.

• Strong clinical assessment and critical thinking skills.

• Excellent communication and patient education abilities.

• Knowledge of nursing practices and home health care standards.

• Ability to work independently and manage time effectively.

• Proficiency with EHR systems.

• Valid driver’s license and reliable transportation for travel within service areas.
