Job Description
Position Summary:
The Care Navigator will work to develop, oversee, and coordinate a Medication Assistance Program (MAP) in a hospital. Facilitate training and serve as the Welvista liaison for the hospital to ensure successful patient enrollment. Manage and implement services aimed at increasing patient enrollment in a medication assistance program. The Care Navigator will serve as primary point of contact for a designated region to facilitate referrals to Welvista programs and services. The care Navigator will Have complete understanding of Welvista's eligibility requirements and processes. Assist in establishing and implementing short- and long-range goals and objectives, and will implement strategies for generating referrals for the organization.
Additionally, the Care Navigator will be responsible for the administrative and operational tasks associated with generating referrals, completing and approving applications and ensuring compliance with federal and state regulations and Welvista policies. The XX will analyze and report data needed to evaluate Welvista's outcomes assessment practices and procedures. Report regional statistics and performance measurements on deadlines. The Care Navigator will provide direct training and education and healthcare providers and must comply with all HIPAA and safety policies, procedures, and regulations. If Welvista is unavailable or not appropriate for patients, the Care Navigator will use all means necessary including but not limited to online pharmaceutical assistance programs, co-pay cards, etc.
Qualifications
Education:
- Bachelor’s degree in Social work (BSW) OR
- License Practical Nurse(LPN)
- Minimum of two years of case management experience required.
- Previous case management experience in health care setting/hospital preferred.
- Current South Carolina (or compact state) licensure as LPN (SCLLR) in good standing required OR License as a Social Worker (LSW) by South Carolina Department of Labor, Licensing and Regulation required.
- Ability to work effectively and collaboratively with nursing colleagues, physicians, department heads, and member of executive leadership.
- Exemplary core customer service skills strongly required.
- Strong organizational skills required.
- Strong PC skills required.
- Strong verbal and written communication skills required.
- Ability to consistently exercise independent judgement required.
- Occasional exposure to difficult patients.
- Physical surroundings generally pleasant and comfortable.
- Prolonged standing and walking.
