ABOUT MIRIAM'S KITCHEN

Miriam's Kitchen (MK) is a mission-driven nonprofit working to end chronic homelessness in Washington, D.C. through meaningful relationships, high-quality meals, and holistic support services. We advocate for permanent supportive housing as a long-term solution while meeting immediate needs through nourishing meals and high-quality social services for individuals experiencing chronic homelessness.

POSITION SUMMARY

The Outreach Case Manager conducts direct outreach to a caseload of high-priority unsheltered individuals, administers VI-SPDAT assessments, assists with document readiness, and supports housing placement by serving as an initial liaison between clients and housing providers across a variety of settings.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Outreach & Case Management

  • Provide outreach, case management, and housing navigation services across all necessary settings, including streets, parks, shelters, hospitals, jails, and encampments.
  • Deliver both 'light-touch' and 'intensive' outreach services based on individual client need.
  • Build rapport with clients using motivational interviewing strategies to support movement through stages of change and acceptance of housing offers.
  • Provide referrals to medical, mental health, and substance use services as appropriate.

Documentation & Benefits Navigation

  • Assist clients in applying for identification documents, entitlements, and other benefits.
  • Collaborate with Miriam's Kitchen's SOAR Specialist to connect clients without income to disability benefits as appropriate.
  • Access the Homeless Management Information System (HMIS) to input VI-SPDAT assessments and upload identification documents.
  • Maintain accurate written and electronic records, case notes, and program documentation while upholding client confidentiality.

Housing Navigation

  • Provide housing match services to a primary caseload of high-priority individuals to facilitate and ensure successful housing placement.

General

  • Attend trainings and professional development opportunities as assigned.
  • Perform other duties as assigned.
Requirements:

QUALIFICATIONSKnowledge, Skills & Abilities

  • Understanding of the structural and systemic causes of homelessness.
  • Familiarity with the homeless services and housing landscape in Washington, D.C.
  • Comfort working with individuals from diverse backgrounds and a wide range of lived experiences.
  • Strong communication skills and professional boundaries.
  • Ability to work both independently and collaboratively as part of a close-knit team.
  • Proficiency with computers, databases, and documentation tools.
  • Ability to navigate Washington, D.C. via public transportation and on foot.
  • Ability to work in physically demanding conditions, including standing and/or walking up to 8 hours per day conducting street outreach in all weather conditions.
  • Ability to lift and/or carry up to 20 lbs.
  • Spanish language proficiency preferred.
  • Valid driver's license and personal vehicle a plus.

Experience

  • Minimum 2 years of experience in services related to homelessness, mental health, substance use, HIV/AIDS, or case management for vulnerable populations.
  • Bachelor's degree, licensure in a relevant discipline, and/or equivalent experience or certification (e.g., Certified Addictions Specialist, Peer Certification).
