South Country Health Alliance is seeking an SNBC Care Coordinator to join our team! In this role, you will be responsible for completing health assessments in a timely manner, developing and implementing care plans for enrollees, and assisting enrollees in achieving their health goals. This position acts as the care coordinator working with an interdisciplinary care team to manage the care and pharmacotherapy of enrollees.
What You’ll Bring:
- Current, valid and unrestricted RN license in Minnesota or bachelor’s degree in social work or in the behavioral science field.5+ years’ experience in a health care delivery setting.
- Minimum of 3 years of clinical experience with at least 2 years of that experience in public health, long-term care, or other settings that cater to vulnerable adult populations A knowledge of both State Public Programs (MN Medicaid) and Medicare (dual) products.
- Two plus years of care coordination experience; county public health or human services contracted care coordination services or other health plan experience is preferred.

SNBC Care Coordinator Position Description

JOB PURPOSE or JOB SUMMARY:
This position is fast paced with a high caseload; responsible for completing health assessments in a timely manner, developing and implementing care plans for enrollees, and assisting enrollees in achieving their health goals. This position acts as the care coordinator working with an interdisciplinary care team to manage the care and pharmacotherapy of enrollees.
QUALIFICATIONS:
Required: Current, valid and unrestricted RN license in Minnesota or bachelor’s degree in social work or in the behavioral science field. Minimum of 3 years of clinical experience with at least 2 years of that experience in public health, long-term care, or other settings that cater to vulnerable adult populations, such as experience working directly with the programs that serve frail members and/or vulnerable adults.
Preferred: Two plus years of care coordination experience; county public health or human services contracted care coordination services or other health plan experience.

ESSENTIAL DUTIES and RESPONSIBILITIES: % of Total Time
- Identify and support enrollees with special health care needs through proactive outreach, data review, and collaboration with county and internal partners. 10%
- Work collaboratively with South Country staff and county partners to coordinate medical services, benefits, and care coordination supports for enrollees.10%
- Participate as a member of an interdisciplinary care team, sharing information to identify and address benefit, utilization, and care coordination issues. 10%
- Assess enrollee needs, unmet needs, strengths, and available supports to develop effective care coordination strategies. 20%
- Develop, implement, and evaluate enrollee-centered care plans in accordance with applicable standards, protocols, and program requirements.10%
- Promote comprehensive, community-based, culturally responsive, enrollee-centered care that is high quality and cost-effective. 5%
- Serve as a primary point of contact and advocate for enrollees by facilitating access to providers, interdisciplinary team members, and community resources.20%
- Participate in committees, work groups, and quality improvement activities to support program development and improvement. 5%
- Coordinate efforts to obtain enrollee feedback regarding their experience with benefits and services and participate in interventions that address identified needs.5%

Maintains satisfactory relationships; frequent interaction to provide detailed procedural explanations or technical responses; may include collaboration in executing established policies, communication with stakeholders (members, providers, county, peers, etc.) is frequent.
Physical Demands
Role primarily involves sedentary work requiring extended periods of sitting, frequent use of computers, phone, and engagement in meetings. The position demands sufficient visual acuity for reviewing detailed regulations and updates, DHS contracts, audit documentation, and other privacy documents, as well as the ability to communicate effectively in person and via digital platforms (computer, phone). The SNBC Care Coordinator must be capable of maintaining focus and accuracy under pressure. Requires high attention to accuracy, precision, details and deadlines

Travel Requirements:
Must be able to travel to counties within our service area for meetings and training an average of 3-5 days per month.
