POSITION SUMMARY

The Clinical Pharmacist Resident provides comprehensive pharmaceutical care to patients at Williamson Health through integrated drug distribution, clinical services, and teaching.

POSITION REQUIREMENTS

Formal Education / Training:

  1. Earned a Doctor of Pharmacy degree from an ACPE accredited college or university
  2. Must have a current license from the State of Tennessee
  3. Must obtain BLS, ACLS & PALS certifications by the end of the Residency program

Workplace Experience:
Hospital or related experience preferred.

Equipment and Skills Training:
Meditech Pharmacy and Order Management Module computer system, Omnicell Workstation, clinical surveillance monitoring system, calculators, personal computers and printers, laminar flow hood, PDAs.

Physical Environment:
Work in an air-conditioned room and/or clean room or office. View and accurately type on a computer terminal for at least a two-hour period. Ability to handle frequent interruptions and adapt to changes in workload and work schedule.

Physical Effort:
View and accurately type on a computer terminal for at least a two-hour period. Competently work in a seated position for a two-hour period. Safely and efficiently stand, stoop, bend, reach and lift in order to maintain the department equipment and supplies.

Occupational Hazards:
Potential for exposure to hazardous and toxic substances (including chemotherapeutic, cytotoxic drugs and cleaning solutions), sticks or cuts by needles and other sharp items. Potential for musculoskeletal injuries if proper lifting and carrying techniques are not used.

Customer Service:
Must practice Williamson Health's Standards of Performance Service Guidelines and Standards. Must be sensitive to the needs of patients, families, visitors, co-workers and staff. Must possess an extremely pleasant, courteous and helpful attitude; excellent communication skills and the ability to work closely and cooperatively with others, including healthcare professionals, is required. Efficiently communicate both orally and in writing. Excellent teaching skills required along with the ability to prioritize and manage both individuals and projects/programs. The ability to communicate effectively with people with varying degrees and types of disabilities. The ability to adjust anticoagulation therapy. Ability to set priorities and solve problems. Fosters a team environment by providing orientation and training to new team members. Ability to meet the pharmaceutical care needs of neonatal, pediatric, adolescent, adult, geriatric, obstetric, and oncology.

PERFORMANCE STANDARDS

  1. Provide operational and clinical support in central pharmacy as determined by departmental need. Review medication orders for accuracy and appropriateness. Order entry for assigned areas. Resolves drug distribution related issues. Resolves Omnicell related issues.
  2. Discuss medication order clarifications with the prescriber, document any changes in patient and pharmacy records, and inform others of medication order changes
  3. Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions
  4. Provide pharmacokinetic consultations and document appropriately.
  5. Prevent, detect, monitor, document, and report adverse drug reactions and medications errors. Participate in the drug therapy management in emergencies situations. Identify and conduct patient safety and process improvement initiatives as need arises.
  6. Promote the use of the formulary by converting non-formulary orders to formulary when possible, but coordinates procurement of non-formulary drugs when necessary. Conduct drug formulary reviews and present to appropriate committees.
  7. Promote and demonstrate rational, cost-effective drug therapy. Identify and implement drug therapy where positive pharma-economic impact could be achieved. The ultimate goal is to develop and assure cost-effective clinical pathways.
  8. Discuss medication, order changes/clarifications with infectious disease physician. Participate in weekly antibiotic rounds and discuss relevant patients to promote antibiotic stewardship.
  9. Document clinical interventions and cost avoidance as part of the departmental documentation program. Monitor the use of expensive medications to assure use consistent with approved criteria. Document in the pharmacovigilance software.
  10. Attend and participate effectively on interdisciplinary and department committees.
  11. Provide effective medication and practice-related education to patients, caregivers, health care professionals, and students.
  12. Assist residency preceptor with preceptorship of student pharmacists on rotations.
