We are seeking a passionate and mission-driven Case Manager to help expand the impact of Shepherd’s House Ministries across all sites and advance this life-changing work.
The Case Manager is accountable for providing consistent, trauma-informed, participant-centered support that promotes safety, stability, and progress toward housing and personal well-being. This role is responsible for engaging participants, assessing needs, developing and monitoring individualized plans, coordinating access to appropriate resources and services, maintaining accurate and confidential documentation, and collaborating effectively with staff, volunteers, and community partners. The Case Manager upholds organizational values, ethical standards, and compliance requirements while contributing to a safe, respectful, and healing environment across all service settings.

Qualifications:
Education & Experience:
- CADC – Certified Alcohol and Drug Counselor or willing to start education towards CADC preferred.
- Bachelor’s degree in Social Work, Human Services, Psychology, Behavioral Sciences, or a related field, or equivalent combination of education and relevant lived/professional experience preferred
- Experience providing trauma-informed case management and support to individuals experiencing homelessness or complex behavioral health and substance use challenges.
- Ability to assess needs, develop service plans, coordinate referrals, and maintain accurate, confidential documentation in systems like HMIS.
- Strong communication, crisis de-escalation, and organizational skills; able to work independently or on multidisciplinary teams with flexible scheduling.

- Work well in a team environment and be able to accept input as well as supervisory guidance.
- Able and willing to interact with shelter guests in a compassionate and respectful manner.

- Able to maintain a positive, professional, Christ-like manner with all donors, staff, guests, and volunteers.
Working Conditions/Physical Requirements:
- Ability to sit, stand, walk, and move throughout facilities and community locations for extended periods
- Ability to transition frequently between administrative tasks (computer/data entry) and direct participant interaction
- Ability to bend, reach, stoop, and lift up to approximately 25 pounds on an occasional basis
- Ability to use a computer, phone, and other standard office equipment for documentation, communication, and reporting
- Ability to travel locally between program sites or outreach locations, as required by the role
- Ability to visually and audibly assess situations, read documentation, and communicate clearly with participants, staff, and partners
