JOB SUMMARY:
Responsible for providing psycho-social rehabilitative services to individuals with severe and persistent mental illness in a Residential Rehabilitation Program.
- Teach and assist members with various activities of daily living skills, (e.g., shopping, budgeting, food selections, safety skills, mobility training, etc.) .
- Maintain a caseload of clients, and complete documentation on time.
- Must be able to work in a team environment.
- Must be able to effectively communicate through written and verbal format.
- Performs case management duties which include the procuring of, and linkage to community resources.
- Ability to drive a 7-12 passenger vehicle.
- Conducts one-to-one and group sessions related to teaching Activities of Daily Living skill.
- Conducts outreach with clients in the community when needed or indicated on the client’s IRRP, in an area conducive to the client’s needs, which may also include shopping establishments, various entitlement establishments, or other outside agencies that facility community integration.
REQUIREMENTS:
- High School Degree/Diploma or equivalent required
- Must have a minimum 2 years of experience working in the mental health field
- Must have a valid Maryland State Driver’s license and be cleared by our Fleet Safety Team to operate LBH-owned vehicles
- Must have a valid BLS Certification from the American Heart Association
