Affirmed Home Care is seeking experienced Per Diem Licensed Practical Nurses (LPNs) to join our exceptional clinical team in FAIRFIELD COUNTY, CT. If you’re passionate about nursing and value building meaningful, long-term relationships with your patients, this opportunity is for you.
About the Role
As an Affirmed LPN, you’ll deliver one-on-one skilled nursing care to medically complex clients in the comfort of their homes. Our cases allow you to focus entirely on your patient—without the fast-paced demands of a facility setting.

- Minimum 1 year of recent nursing experience (acute care or home care strongly preferred)
- Active Connecticut LPN license
- Current BLS certification
- Eligibility to work in the U.S.
- Valid driver’s license
- Physical exam within 12 months
- PPD or QuantiFERON within 1 year, or chest X-ray within 5 years
