JOB SUMMARY

The Quality Assurance Coordinator supports the Director/Administrator in developing and administering the Quality Assessment/Performance Improvement (QAPI) programs. This role is responsible for planning, organizing, coordinating, and directing the facility’s Quality Assurance Program to ensure the highest standard of patient care and safety in compliance with federal, state, and local standards and regulations.

RESPONSIBILITIES

  • Adhere to established Home Health and Hospice policies and procedures.
  • Promote and understand the principles of continuous QAPI.
  • Participate in the orientation of new staff to the Home Health and Hospice QAPI program.
  • Support staff in planning and consultation needs related to quality initiatives.
  • Assist in the development and implementation of policies and procedures in compliance with Medicare, Medicaid, accreditation standards, and local/state laws.
  • Participate in administrative functions related to quality programs as assigned.
  • Serve as a role model for colleagues, demonstrating high standards in dress, conduct, cooperation, and job performance.
  • Maintain confidentiality of all patient-related information.
  • Take responsibility for regular attendance and punctuality, meeting job requirements as necessary.
  • Act as a resource for employees regarding quality assurance.
  • Assist in investigating and reporting issues related to patient care and policies.
  • Immediately report accidents, incidents, lost items, or unusual occurrences to the Director or Clinical Manager.
  • Attend relevant continuing education programs beyond routine in-services and share information with staff.
  • Develop QAPI activities with data collection, aggregation, analysis, action-taking, and results reporting in line with the Home Health and Hospice QAPI plan.
  • Review patient medical records for compliance with relevant laws, accreditation standards, and agency policies.
  • Chair the QAPI committee, prepare QAPI reports and minutes, and forward reports as per the QAPI plan.
  • Attend monthly Quality Assurance Committee meetings to review assessment tools, data collection reports, and activities related to quality assurance across departments.
  • Assist other committees and departments in developing monitoring tools, standards, and methodologies to enhance patient care and safety.
  • Implement recommendations from the Quality Assurance Committee as they relate to the quality assurance program.

QUALIFICATIONS

  • Minimum of two (2) years of college education.
  • Experience in clinical healthcare, with direct experience in quality assurance preferred.
  • Strong understanding of quality assessment and assurance procedures.
  • Ability to make timely, independent decisions when circumstances require.
  • Leadership and supervisory abilities with a collaborative, team-oriented approach.
  • Effective communication skills and the ability to work harmoniously with personnel, residents, families, visitors, and government agencies.

WORKING ENVIRONMENT

Primarily office-based with intermittent movement and subject to frequent interruptions. Involvement with personnel, residents, family members, and other stakeholders as required, including emergency call-backs.

JOB RELATIONSHIPS

Reports to: Quality Assurance Director

RISK EXPOSURE

Moderate risk due to frequent interaction with patients and personnel in clinical settings.

LIFTING REQUIREMENTS

The role may require occasional light lifting of up to 20 pounds, along with intermittent standing, sitting, and movement throughout the office. The ability to work extended periods while sitting or standing as needed is essential for fulfilling daily responsibilities.
