About This Position
Position: Licensed Vocational Nurse/Licensed Psychiatric Technician
FLSA Status: Non-exempt( hourly)
Reports To: Registered Nurse/QIDP/Executive Director
Normal: All Shifts available, including weekends
SUMMARY
The LVN/LPT staff will work with individuals with developmental disabilities in a community based, residential facility, in the medical and health functions of each client in the facility(s) assigned and will perform all necessary duties pursuant to the job responsibilities. Successful completion of LVN/LPT programs with CA issued license.
JOB DESCRIPTION
An LVN/LPT shall perform duties as directed by posted duty list and from Facility RN/Manager/QIDP/Executive Director. All duties are expected to be completed successfully to meet performance requirements.
• Responsible and competent in following active treatment plan and implementing goals and objectives from ISP (Individual Service Plan).
• Assist the Facility RN, Physician and other health professionals including but not limited to keeping close contact and frequent communications in updating clients’ status as needed.
• Ensuring that the DCS are capable and competent in completing nursing/health related tasks appropriately and accordingly.
• Provide direct care duties to all clients
• Responsible to know the client program plans and activity schedule.
• Must learn and understand Clients Rights.
• Responsible for documentation in all required areas within the medical program book for follow through of treatment and data collection.
• Responsible to report any unusual occurrence.
• Responsible as a Mandated Reporter related to the abuse policy.
• Responsible for house cleaning duties including but not limited cooking
• Meal are considered “on duty”. Employees are not relieved from their duties to have their meals and agree to an on-the job paid meal period.
• Adhere to the Company’s Standard of Conduct and Policies and Procedures according to local, state and federal regulations.
• Follow additional directives from RN/QIDP/Executive Director when applicable.
You are also required to attend in-service training 3 hours per month and other staff meetings to gain further knowledge of programs and operations. LVN are to ensure the safety for clients and homes must be maintained at all times, safety checks are routinely done. As well over see the maintenance and care of all medical equipment and supplies, is required.
ESSENTIAL DUTIES
DAILY:
1. In the beginning of the shift, licensed staff will read communication book to ensure continuance of care and gain understanding of all clients’ condition.
2. Licensed staff to ensure all medications were administered through checking the bubble packs of all clients and review the Medication Administration Records (MAR)
3. Any discrepancy on the bubble pack and MAR are reported to Primary Care Physician, RN and QIDP.
4. Medication passes to all clients on timely manner. Read parameter before giving any medication i.e check blood pressure prior giving hypertension medication.
5. Make medical appointment as needed and document on “Medical Appointment” folder.
6. Take clients to medical appointment.
7. Follow up on and implement change of orders and recommendations from medical appointment.
8. Ensure daily body check for all clients, any findings, please follow the Staff Report Procedure
9. Monitor staff as indicated or as needed during medication pass and or mealtime.
10. Help staff as needed with ADL’s, implementation of plan(s) and interaction between staff and clients.
11. Daily charting on each client, document any acute health issue/problem daily until resolved.
12. Check medication cabinet, ensure the external and internal medication are stored separately, check for expired medication, check for medication supplies.
13. Order re-fill medications to the pharmacy when medications are low.
14. Communicate with RN for any change of client condition and/or any issue related to medical and health matters.
15. Chart daily and as needed
WEEKLY:
1. Complete weekly summary
2. Check all physician orders for the need of lab, immunization or other consultation.
3. Check MAR Review if any weekly orders i.e client’s weight are followed through
4. Address acute and on-going health issues, including but not limited to Staff Report documentation
5. Check the need of house supplies, submit supply order form
MONTHLY:
1. Review and assure accuracy of MAR, weight, vital sign, etc.
2. Prepare MAR and Physician Order for upcoming months. MAR’s must be audited and sent back to
Pharmacy for corrections, if needed.
3. Cycle medications must be checked and corrected.
4. Ensure that day program has a copy of current physician order and MAR if medication is being given at the day program.
5. Ensure that all Physician orders are signed my doctor every month.
6. Ensure that all applicable forms/logs are up to date such as seizure record, vital sign, weight, infections, etc.
7. Prepare and present training to the staff (as needed)
QUARTERLY:
1. Ensure recommendations from health/medical professional are being carried through.
2. Review and update Immunization Record.
AS NEEDED:
1. Take orders from the physician and/or RN.
2. Files all health care documents in each of client’s chart.
3. Reviews medical appointments findings and recommendations with RN.
4. Assist RN in NATP training as needed.
5. Assist RN in ensuring all clients medical and health needs are met.
MINIMUM QUALIFICATIONS
• Must be 18 years of age.
• Current CA LVN/LPT Licenses
• Current Driver's License or Identification Card
• Documentation proving eligibility to work inside the USA
• Clear Criminal History
• Clear Pre- Employment Physical and Drug Screen
• Ability to work in a group home, home-like setting
• Ability to communicate (verbally and written) with all levels of personnel, internal and external
PHYSICAL DEMAND
Ability to walk, stand long hours, bend, squat, push and lift frequently. Ability to lift up to 50 pounds occasionally. Ability to assist an individual with unsteady gait. The Company enforces the employment provisions of the (ADA) Americans with Disabilities Act.
WORK ENVIRONMENT
May occasionally walk on slippery or uneven surfaces.
Will work with individual(s) with aggressive and verbal abusive behaviors
May occasionally drive/Assist other staff and managers with clients to their community outings, doctor’s appointment and/or hospital/ER visit.
