About the Role
Touchstone Home Health is seeking a Speech Language Pathologist (SLP) to join our San Antonio team—where clinical excellence meets purpose-driven care. Key Responsibilities
- Provide speech therapy services in accordance with physician orders and individualized care plans
- Conduct comprehensive evaluations of communication and swallowing needs
- Develop and adjust treatment plans to support patient progress and safety
- Educate patients, families, and caregivers on home-based strategies and exercises
- Complete timely and accurate documentation, including OASIS assessments and care updates
- Collaborate with physicians and interdisciplinary teams to coordinate high-quality care
What We’re Looking For
- Master’s or Doctoral degree in Speech Language Pathology
- Current SLP license in the state of Texas (or eligibility)
- Strong communication skills and the ability to connect with diverse patients and families
- Adaptability in dynamic, home-based care environments
- Organized, self-directed approach to managing schedule and documentation
- Willingness to travel between patient homes throughout the San Antonio area
