True Health is seeking a Physical Therapist to work side by side with physicians, health coaches, and the broader care team so patients get consistent guidance and real follow-through.

What You'll Be Doing

You deliver specialized physical therapy care that fits into a coordinated, patient-centered plan.

· You deliver evidence-based care and coordinate it across the team

· Evaluate patients through history review, movement assessment, and musculoskeletal screening to find root causes and barriers.

· Build and carry out individualized treatment plans aligned with physician-directed goals and the broader care plan.

· Track progress and outcomes, then adjust treatment approaches to keep care effective and patients engaged.

· Share findings and recommendations with physicians and the interdisciplinary care team to keep care connected.

You guide exercise progression and movement habits patients can sustain

· Coach patients to start, progress, and maintain physical activity that matches their health status, fitness level, and care goals.

· Partner with health coaches on exercise and movement recommendations that support long-term activity habits.

· Teach body mechanics, posture, injury prevention, and safe exercise progression in clear, practical ways.

· Support patients moving from low activity to structured, goal-oriented routines through graduated programming.

You treat mobility limitations and musculoskeletal conditions with hands-on skill

· Assess and treat mobility limitations, musculoskeletal pain, and conditions that interfere with exercise and lifestyle medicine programming.

· Provide manual therapy, therapeutic exercise, and neuromuscular re-education when clinically appropriate.

· Recommend assistive devices, home exercise programs, and activity modifications that support safety and independence.

· Recommend and administer treatments such as dry needling or shockwave therapy, and support bundled solutions that include these treatments.

You show up as an embedded specialist in a concierge care model

· Participate in care team consultations and patient care conferences to integrate goals into each patient’s overall health plan.

· Communicate consistently with physicians, health coaches, and other team members about progress, modifications, and clinical concerns.

· Contribute to integrated care protocols connecting Lifestyle Medicine, nutrition, and physical therapy services.

· Document accurately and on time in the EMR, following True Health standards and HIPAA requirements.

You deliver a patient experience that feels warm, professional, and personal

· Bring a hospitality-centered approach to every patient interaction, consistent with True Health’s concierge standards.

· Help patients understand the value of physical therapy within their overall wellness journey.

· Make clinical decisions with a patient-first mindset that prioritizes safety, dignity, and individualized care.

What You Bring

You're someone who:

· Makes patients feel safe and respected, even when they are in pain, anxious, or discouraged.

· Can hold a high clinical bar while still meeting people where they are and building trust over time.

· Communicates clearly with both patients and teammates so care stays coordinated and no one is guessing.

· Follows through, documents on time, and keeps details tight because continuity of care depends on it.

· Works naturally in a team-based model and stays open, consistent, and collaborative day to day.

Experience that helps:

· Doctor of Physical Therapy (DPT) degree from an accredited program.

· Active and unrestricted Physical Therapist licensure in the state of Illinois.

· 2+ years of clinical physical therapy experience in an outpatient, clinical, or healthcare setting.

· Demonstrated experience or specialized training in pelvic floor physical therapy.

· Proficiency with EMR systems and clinical documentation standards.
