About This Position
About the Role
We are seeking a compassionate, skilled Occupational Therapist (OT) to join our growing home health team. In this role, you will provide patient-centered occupational therapy services in the home setting, helping patients regain independence, improve functional abilities, and safely remain in their homes.
This position is ideal for an OT who values autonomy, flexibility, and meaningful one-on-one patient care while maintaining high clinical and documentation standards.
Key Responsibilities
- Perform comprehensive occupational therapy evaluations in the patient’s home environment
- Develop, implement, and update individualized plans of care based on physician orders
Provide therapeutic interventions to improve:
- Activities of Daily Living (ADLs)
- Instrumental Activities of Daily Living (IADLs)
- Upper extremity function
- Cognitive and safety awareness
Educate patients and caregivers on:
- Adaptive equipment
- Energy conservation techniques
- Home safety and fall prevention
Collaborate with the interdisciplinary care team (RN, PT, ST, HHA, DON)
Complete accurate and timely clinical documentation in accordance with agency policy and regulatory standards
Participate in care conferences, audits, and quality improvement initiatives as needed
Maintain compliance with Medicare, Medicaid, and accreditation standards
Qualifications:
- Graduate of an accredited Occupational Therapy program
- Current state OT license in good standing
- Minimum 1 year of experience preferred (home health experience a plus)
- Strong clinical assessment and critical-thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Valid driver’s license, reliable transportation, and active auto insurance
- Current CPR certification (or willingness to obtain)
