Job Description

Summary

Supports patient/family needs through an advocacy role in order to effect change in behavior and provide biopsychosocial intervention as necessary. Provides assistance to the Case Coordinator and Healthcare Team, crisis intervention, complex or long-term placement linkage to appropriate community resources. Works directly with staff to facilitate achievement of quality and cost effective patient outcomes in Rehab.

Education

Master's Degree in Social Work.

Certification

Must have current Indiana LSW or LCSW license.

Experience

Must have worked in a setting performing essential hospital or healthcare social work functions, such as counseling, psycho-social assessment, crisis intervention, etc. for two to three years. Must have experience in using a computer related to necessary applications.

Other Qualifications

Must be able to work flexible hours.
