Corrections Linkage and Education Coordinator in Albany, New York at Alliance for Positive Health

The CLEC works with individuals living with HIV and/or Hepatitis C who are incarcerated and/or preparing to return to the community. The CLEC works to motivate and ensure engagement and adherence in treatment, education, prevention, and support services. The CLEC also facilitates Peer Education training and support, targeted recruitment, condom promotion and education, and linkage to testing, treatment, and prevention with incarcerated individuals in designated state correctional facilities. Work with individuals who are living with HIV and/or Hepatitis C who are incarcerated and/or returning to the community in order to motivate and ensure engagement and adherence in HIV and/or Hepatitis C-related treatment, prevention and support services. Service targets will vary by correctional facility; assigned interventions will be outlined in annual workplans.
