Job Summary: We are seeking a compassionate and reliable CNA to provide high-quality, patient-centered care in a home health setting. In this role, you will support patients with daily living activities, promote safety, and work collaboratively with an interdisciplinary care team.

Essential Functions:

  • Assist with activities of daily living (ADLs), including bathing; grooming; oral hygiene; dressing; feeding and mobility assistance; transfers and ambulation support.
  • Follow and implement the patient’s plan of care under the direction of the Case Manager.
  • Provide skin care, nail care, foot care, and pressure relief to maintain patient comfort and safety.
  • Take and accurately document vital signs in the EMR; report abnormal findings to licensed staff.
  • Monitor and report changes in patient condition, including pain or safety concerns.
  • Ensure proper positioning and body alignment for patients.
  • Maintain a safe environment to reduce the risk of injury or health complications.
  • Educate and support patients with daily routines and care needs.
  • Collaborate with nurses, therapists, and other care team members.
  • Complete timely and accurate clinical documentation.
Requirements:

Professional Requirements:

  • Maintain HIPAA compliance and patient confidentiality at all times.
  • Adhere to all state, federal, and agency regulations, including supervisory visit requirements.
  • Follow infection control, safety, and quality assurance protocols.
  • Attend required trainings, in-services, and annual evaluations.
  • Maintain 12 hours of continuing education annually.

Qualifications:

  • Active CNA or Home Health Aide certification (state-approved program required).
  • Knowledge of basic nursing care and patient safety practices.
  • Strong communication and teamwork skills.
  • Ability to manage time effectively and maintain accurate documentation.
  • Basic computer skills and familiarity with EMR systems required.

Physical Demands

  • Prolonged standing and walking required.
  • Ability to lift, move and transfer patients weighing over 50 lbs.
  • Ability to identify patient needs, make quick decisions, and work under stressful conditions to meet deadlines.
  • Meet patient/family psychosocial needs.
  • Requires hand-eye coordination and manual dexterity.
  • Ability to utilize durable medical equipment in a home setting.
