In this role, youll work closely with clients and their families to understand their needs, create tailored care plans, coordinate services and make sure our clients continues to meet their goals as circumstances change.

Key Responsibilities

• Build strong relationships with clients and families throughout their care journey.
• Complete assessments and develop personalised care plans that reflect each clients goals.
• Coordinate services and work with caregivers and health professionals to deliver consistent, quality care.
• Monitor client wellbeing, review care needs and adjust support where required.
• Maintain accurate records and ensure services meet aged care quality and compliance requirements.
• Demonstrate resilience and embraces change with a positive, solutions-focused mindset.

About You

• Demonstrated experience working within the Support at Home (SaH) program and a strong understanding of its requirements (Essential)
• Strong relationship building skills and the ability to work well with clients, families and stakeholders.
• Exceptional administration skills, with the ability to efficiently manage tasks using Microsoft Office programs.
• A practical, team-focused approach, with the confidence to work independently when needed.
