Community Health Worker at Holyoke Medical Center – Holyoke, Massachusetts

We are seeking a Community Health Worker who, under the general direction of the Senior Community Navigator and Manager, collaboratively provides comprehensive care management services in response to e-referrals, and assists patients/clients to navigate and access community services that will have a positive impact on their overall health.

REQUIREMENTS: High School Diploma or equivalent; bi-lingual or multi-lingual preferred; driver's license and vehicle; must complete smoking counselor training within one year of hire; completion of CHW Core Competency Education in Massachusetts; trained and certified in chronic disease management such as asthma, hypertension, diabetes and mental health; pediatric experience a plus
