Position Details:

• Location: Troy office, with a hybrid schedule.
• Hours: Monday to Friday, 8:30 am to 5:00 pm.
• Additional Requirements: Weekend backup and weekly rotation of evening on-call duties (calls only).

Responsibilities:

• Receive and review case referrals.
• Assess patient needs and assign appropriate clinicians.
• Provide guidance and support to ensure quality home care services.
• Collaborate with the team to establish therapeutic goals and develop comprehensive care plans.
• Conduct quarterly record reviews and make recommendations for improvement.
• Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals.

Qualifications:

• Registered Nurse with current licensure in Michigan.
• Minimum 2 years of management or supervisory experience in healthcare, preferably in home care.
• Current CPR certification.
