Cardiac Rehabilitation Nurse in Fairfield, Iowa at Jefferson County Health Center

POSITION OVERVIEW (non-exempt):

Responsible for administering the Cardiac Rehab programs: Phase I, II, III. Responsible for the planning, the development, and the daily tasks of the Cardiac Rehab programs.

QUALIFICATIONS:

• Graduate of accredited school of nursing, with current Iowa RN license.
• BCLS and ACLS required.
• Successful completion of JCHC pharmacology qualifying examination.
• Some knowledge of teaching methods.

Reports to Cardio-Pulmonary & Support Services Director

DIRECT REPORTS:

None

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Assists in formulation of departmental goals and design, implement, provide, and evaluate nursing care in all phases of Cardiac Rehab.
• Assists in formulation and revisions of the department specific policies and procedures.
• Responsible for ensuring all patients have an initial individualized treatment plan (ITP), a 30 day ITP, a 60 day ITP (if needed), and a final ITP.
• Responsible for the development, scheduling, implementation and documentation of patient education.
• Actively recruits and manages all aspects of the Cardiac Rehab program including scheduling, patient education, patient monitoring, exercise sessions, coordination of other depts. involved, documentation, and coordination of utilization review process. Participates as needed in the billing process for Cardiac Rehab.
• Participates on designated committees, auxiliary and community outreach programs.
• Assists in preparing, recommending and administering budgets to achieve stated goals in Cardiac Rehab.
• Responsible for continuous quality improvement activities of Cardiac Rehab program. Participates in hospital wide QI projects by abstracting information on core measures.
• May be required to perform other duties as designated by manager/supervisor. Regular and punctual attendance.

POSITION-SPECIFIC REQUIREMENTS:

• Demonstrated competence in patient/disease management and possess leadership skills. Computer skills required.

WORK ENVIRONMENT:

• Works in a climate-controlled environment
• May come in contact with hazardous chemical or treatment modalities
• Possibility exists of exposure to communicable diseases, blood/body fluids, chemicals, and gases
• Moderate to loud noise environment when machines/equipment in use
• Involvement in checking in/moving of supplies may result in unavoidable work-related injury
• Must be constantly alert for patient/family emergency situations
