Job Description

Description:

Position Summary: The Outreach and Enrollment Specialist is responsible for identifying and engaging individuals eligible for Health Homes Care Management Services and promoting the services effectively to obtain clients’ agreement to be referred for program enrollment. The ideal candidate will have a strong background in substance use disorder services, experience engaging individuals with complex behavioral health and medical needs, and the ability to build trusting relationships with individuals who may be hesitant to participate in services. In addition to clinic-based outreach and enrollment, the Specialist will participate in broader community outreach, referral development, and other activities designed to increase program enrollment and strengthen community partnerships.

Essential Functions

  • Establish and maintain a strong working relationship with SUD treatment staff to support the identification and referral of potentially eligible individuals.

  • Provide orientation, intake, and screening for all referred individuals. Complete all required enrollment, screening, assessment tools, and consent forms mandated by the HealthHome and/or NADAP Policy & Procedure.
  • Collaborate with clinic personnel and Health Homes Care Management staff to facilitate warm handoffs and a smooth transition into services.
  • Participate in outreach activities at hospitals, community-based organizations, treatment programs, shelters, and other locations serving individuals who may qualify for Health Homes services to meet referral targets
  • Maintain accurate and timely documentation of outreach contacts, referrals, enrollment activities, and outcomes.
  • Track outreach activities, referrals, enrollment progress, and barriers to engagement.
  • Attend community meetings, provider events, and other outreach opportunities as assigned.
  • Uphold all HIPAA regulations and confidentiality protocols to record each interaction with every client served (including demographic and diagnostic information).
  • Provide regular updates to management regarding outreach efforts, enrollment outcomes, and opportunities for improvement.
  • Ensure compliance with program policies, documentation standards, confidentiality requirements, and applicable regulatory guidelines
  • Perform other related duties as assigned.
