About the RoleHeritage Behavioral Health Center is seeking a dedicated Community Support Specialist (Case Manager) to join our Community Support Team (CST). CST provides mental health rehabilitation services designed to decrease hospitalization and crisis episodes and to increase community functioning so individuals can achieve their rehabilitative, resiliency, and recovery goals.
Working under the clinical direction of a full-time QMHP team lead, the Community Support Specialist delivers individualized, community-based services that facilitate illness self-management, skill building, and connection to community resources — in accordance with Illinois Rule 132/140 requirements.

| Bachelor's Degree in human services, social work, education, or related field | $25.00/hour | Every other Friday off — paid |
- Provide individual community support services to adults with serious mental illness in their homes and communities
- Help individuals develop illness self-management skills, including symptom recognition, medication routines, coping strategies, and relapse prevention
- Deliver skill-building interventions to strengthen daily living, social, problem-solving, and independent living skills
- Support individuals in identifying and strengthening natural supports, including family, friends, and community connections
- Connect individuals to community resources including housing, employment, benefits, medical care, and social services
- Contribute to individualized, person-centered treatment plans that reflect each person's recovery goals
- Monitor and document progress toward treatment plan goals in the electronic health record
- Collaborate with internal and external providers to support continuity of care
- Participate in interdisciplinary team meetings and clinical staffings
One of the following is required:
- High School Diploma or GED plus 5 years of documented supervised experience in behavioral health or human services
- Bachelor's Degree in human services, social work, education, psychology, counseling, or a related field
All candidates must have:
- A valid driver's license, reliable transportation, and proof of current automobile insurance
Preferred:
- Experience providing community-based behavioral health, case management, or rehabilitation services to adults with serious mental illness
- Certification such as CRSS or CADC
- Every other Friday off — paid (built into your schedule)
