At Children’s Services of Roxbury, we’re not just offering a job, we’re offering a chance to be part of something bigger. Our mission is to bring peace of mind to children and families across Massachusetts. We foster a culture of trust, humility, and respect, where every team member is valued and supported. If you’re looking to make a real impact and grow with a team that strives to create a brighter future for children and families across Massachusetts, this role is for you.

Position Summary:

Children’s Services of Roxbury (CSR) partners with the Department of Children and Families (DCF) to provide intensive, home-based Support and Stabilization Services to parents, caregivers, and their children. The goal of these services is to strengthen family functioning, enhance child safety and well-being, and support families in achieving identified goals while maintaining safe, stable, and nurturing home environments.

CSR provides individualized, family-centered services that address a wide range of needs, including parenting education and support, behavioral health and medical care coordination, community resource navigation, educational and vocational assistance, and advocacy related to legal matters such as domestic violence, probation requirements, and Child Requiring Assistance (CRA) proceedings. Services are tailored to each family's unique strengths and challenges and are delivered in collaboration with DCF and other community providers.

Family Stabilization & Support Clinicians conduct one to two home visits per week, as determined by the referral and family needs. Clinicians provide ongoing assessment, crisis intervention, advocacy, service coordination, and goal-focused support throughout the duration of the contract period. Regular communication and collaboration with DCF staff and other service providers are essential to ensure coordinated care and successful outcomes for families.

Essential Duties and Responsibilities:

Direct Service Delivery

  • Conduct weekly home visits with assigned families, including evening hours as needed to accommodate family schedules.
  • Provide family-centered support, advocacy, resource coordination, and skill-building services.
  • Assist families in accessing community resources, healthcare, educational, vocational, and behavioral health services.
  • Respond to family crises, hospitalizations, emergency meetings, school meetings, and other urgent situations as needed.
  • Utilize a strengths-based and culturally responsive approach to support family stability and goal achievement.

Case Management and Documentation

  • Maintain accurate, timely, and organized case records in accordance with agency, contractual, and regulatory requirements.
  • Complete detailed case notes and home visit narratives in the Credible system following each client contact.
  • Document all telephone calls, collateral contacts, and service-related activities in a timely manner.
  • Prepare and submit monthly reports and other required documentation within established deadlines.
  • Develop, implement, and monitor treatment plans through the Virtual Gateway system, ensuring activities align with goals identified by DCF and the family.
  • Complete progress reviews and treatment plan updates at required intervals.
  • Maintain confidentiality and ensure all records are accurate, complete, and audit-ready.

Collaboration and Professional Engagement

  • Maintain regular communication and collaboration with DCF social workers, service providers, schools, healthcare professionals, and other community partners.
  • Participate in DCF case conferences, team meetings, and other multidisciplinary planning meetings.
  • Attend and actively participate in weekly supervision with the Program Director.
  • Represent Children's Services of Roxbury professionally in all interactions with families, colleagues, referral sources, and community partners.
  • Maintain appropriate professional boundaries with clients, families, and service providers.
  • Contribute positively to team culture through professionalism, flexibility, and collaboration.
Qualifications
  • Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or a related field required; Master’s degree preferred.
  • Minimum of one (1) year of experience providing case management, family support, counseling, or related services to children, adolescents, and families.
  • Bilingual candidates are strongly encouraged to apply.
  • Valid driver's license, reliable transportation, and proof of automobile insurance required.
  • Ability to work flexible hours, including evenings and occasional early morning appointments, based on family needs.
  • Strong written and verbal communication skills.
  • Ability to manage multiple priorities and complete documentation within required timelines.
  • Successful completion of all state and federal background checks.
